Nominees in all sports categories must be a FHHS graduate, ten years removed from high school, played a varsity sport
for at least two years, achieved All conference status or better.
Nominees in the coaches category must be 5 years removed from FHHS. All nominees are ENCOURAGED TO BECOME A PAID MEMBER OF THE FHHS ALUMNI ASSOCIATION prior to their HOF Induction at https://fhhsaainc.com/membership-info/.

* Persons are not permitted to nominate themselves.
* NO LATE SUBMISSIONS or WORD OF MOUTH applications will be accepted.
* Candidates or teams NOT selected for induction will automatically be considered for acceptance during the next calendar year.

Report Submission

1. Nominator must complete the Fairmont Heights High School Athletic Hall of Fame (FHHSAA AHOF) on-line Nomination Form found at https://www.fhhsaainc.com/athletic-hall-fame and submit it electronically no later than midnight, March 1, 2025. All completed nomination forms must be submitted by email to athleticHOF@fhhsaainc.com, no exceptions.

2. Nominee must approve being nominated before the committee will process the form. If nominator need more space to support their nominee, they should provide supplemental pages. These pages should have the nominee’s name at the top of each page.

3. Please upload supporting documentation of any accomplishments, such as, All Metropolitan or newspaper article.

4. All information on this form is confidential and will only be shared with the committee members of the FHSAA AHOF.

3. Teacher – Responsible teacher for the sport program, may not be the sponsor
4. Legacy – Student athlete over 50 years since graduation. Outstanding performances in their respective field, but did not receive any recognition
5. Team/Squad Recognition: Ex: 1959 County Track Champions, 1967 Varsity Cheerleaders, Basketball State Champions
6. Team Support (Manager, etc.) - Manager of the team, if nominated
7. Team/Squad – Year of the entire squad that is nominated

b. Sports
1. Choose the sport or sports that the nominee is being recommended for, if more than one select all that apply. They must be an elite athlete.
